HIPAA notice of privacy practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
1. Our duties
Pillar Health Co. acts as a Business Associate of our clinical partners — the independent providers, medical groups, and pharmacies contracted through our provider network, Architek Health — who are Covered Entities under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"). We are required by law to maintain the privacy of your protected health information ("PHI"), give you this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.
2. How we may use and disclose PHI
- Treatment: to provide, coordinate, or manage your care between clinicians, pharmacies, and labs.
- Payment: to process payments and verify eligibility.
- Health-care operations: quality assessment, credentialing, training, and customer service.
- As required by law: public health, mandated reporting, judicial orders, and law enforcement subject to HIPAA.
- With your authorization: for any other use, including most marketing or sale of PHI. You may revoke an authorization in writing at any time.
3. Your rights
- To access and obtain a copy of your records.
- To request a correction of records you believe are inaccurate.
- To request restrictions on certain uses and disclosures.
- To request confidential communications by alternative means or location.
- To receive an accounting of certain disclosures.
- To receive a paper copy of this notice on request.
- To file a complaint with us or with the US Department of Health and Human Services without retaliation.
4. Breach notification
We will notify you in the event of a breach of unsecured PHI in accordance with HIPAA and applicable state law.
